St Jude’s South West Capacity Building Program

We are excited to announce St Jude’s Disability Services has been successful in securing a grant from the National Disability Insurance Scheme (NDIA) through the Information, Linkages and Capacity Building (ILC) program. The aim of the Mainstream Capacity Building (MCB) program is to stimulate and encourage change in the delivery of mainstream services through the development and trialling of new approaches that are more inclusive and welcoming for people with disability.

Our MCB program will be carried out in the South West of WA, as living in the remote South West area of Western Australia presents many challenges for those living with disability, in particular for people with psychosocial disability. This program will engage an Allied Health Consultant to deliver a ‘map and gap’ exercise which identifies roadblocks to service accessibility. It aims to break down the barriers to successful long-term transition back into the community for people with disability leaving hospital care.

The project will create and implement a strategic plan which offers practical solutions to overcome barriers, provide support with all stages of transition, forge stronger links between existing services, and support individuals living with disability to access services available under the NDIS. We will collaborate with the Department of Health, APM and other community organisations in the region to effectively implement the program.
