Community In-Home Support

St Jude’s experienced and diverse Support Team provides NDIS registered in-home support services to children and adults. This support is designed to assist or supervise you with day-today tasks in the home to work toward building your independence. 

We work closely together to understand you, your interests and your goals,  to build your individual skills so you can live as independently as possible and take part in the activities you enjoy. 

St Jude’s recognises there is no one size fits all approach when it comes to providing in-home support. This is why we ensure our team is as diverse as it is experienced, with compatibility with clients of utmost importance.

Some of the supports St Jude’s can offer you include, but are not limited to, individual care, health, nursing, and domestic supports such as cleaning, washing, cooking, and gardening assistance. We tailor these supports to your individual preferences and requirements.

St Jude’s put you at the centre of our services. In addition to weekday services, we are available to support you both after hours and on weekends. We also do not require minimum NDIS funding, service all suburbs in Perth and WA’s South West and have support workers available to immediately commence supporting you and building your skills in the comfort of your own home.

Services Offered

  • Personal Care and Support such as showering, dressing, toileting, grooming and personal administration such as paying bills
  • Domestic Support such as cleaning, washing and home maintenance
  • Gardening
  • Nursing such as first aid, wound care, and the management of medication, continence, behaviour and seizures
  • Meal and Food Support  including meal preparation and grocery shopping
  • Use of  Assistive Technology such as aids and appliances, hearing and communication devices
  • Additional Services – You can also access services including community participation, therapy, accommodation and support coordination.

Funding

All our providers are registered with the NDIS, if you have a funded plan you are eligible for St Jude’s Disability Services.

If you do not currently have an NDIS plan, but you are living with a disability and could benefit from assistance in your daily life, our dedicated team can guide you through the application and planning process. 

Our services can also be accessed via Medicare or private funding.
