St Jude’s Home Care (Aged Care) services assist you to stay in your own home for longer and avoid moving to a nursing home by providing you with the support to live independently.

To access our services, it is recommended that you have Home Care Packages to receive subsidised care services. If you do not have Home Care ‒ don’t worry we may have cost-effective alternative options for you. Home care can help you to achieve this by giving you extra assistance to stay safe, comfortable and well at home.

Asking for extra help to manage at home can be difficult – but it’s more common than you might think. Home care is helping almost 1 million Australians to age well in their own homes. If you’re considering home care, you have different options to choose from, including a Home Care Package. To make an informed decision, you’ll want to know what’s available and what it costs.

There are different types of home support available. The option you choose will depend on the level of care you need, how long you need it for, and whether you want to rely on government-subsidised services, private services or help from your personal networks. The main options are:

  • basic or short-term support through the Commonwealth Home Support Program
  • more or longer-term support with a Home Care Package
  • private services
  • personal networks and community.

Some people use a combination of these options to meet their needs.

Need More Information?

Home Care provides you with a wide range of support services that help you, or the person you care for, to continue living at home. You can receive home care as little as 30 minutes a day for a range of support services from domestic support to complex nursing and therapy services.

You may eligible for government supported Home Care which means that you may not need to pay anything. Speak to your GP about your support needs.

The GP can refer you to an Aged Care Assessment Team, or if you are in hospital the medical and nursing staff can arrange this for you, or you can ring ‘My Aged Care’ on 1800 200 422.

If you do not have a GP or in a crisis situation give us a call on 9279 4343.

The Aged Care Assessment team may meet with you in person at your home. With your consent they will assess your care needs and eligibility for services. They will then work with you to develop a support plan which addresses your needs and preferences.

You will receive a letter outlining your eligibility for a package and approved the level of package and funds that has been allocated.

Once you have received the results of your assessment, you can contact us to discuss the services you would like to receive.

We can also arrange top-up services if the funded support does not meet your support needs.

Our representative (Case Manager) will come to you and discuss the services we can offer. If you choose us, we will carefully design a plan that best suits you. You may ask your friends or families to join the meeting.

With the information you provided, the Case Manager will prepare a service plan, proposed budget and a home care agreement.

These will be given to you for you to discuss with your family, carer or your friends to make sure you have everything you need.

When you are ready give us a call and the Case Manager will return to meet with you to sign your Home Care Agreement, complete a full assessment, prepare your care plan based on your assessment, and confirm your service plan.

We will provide you a file with all relevant information to keep at your home.

Our schedulers will then put in place and assign your services based on the dates and times you wish to commence with St Jude’s Home Care Services.

Self-managed OptionSelf-managed home care allows you to have more control over your services. The below are some of the examples of choices that you may choose with self-managed option.

  • Search and hire your own support worker
  • Choose services who you believe is most suitable and contact them directly
  • Communicate your needs and how you want to be supported
  • Discuss and agree on a rate
  • If you are satisfied with your choice of support worker, hire them through us
  • Schedule your weekly services
